A Marvel fan-made video imagines Old Man Steve Rogers (Chris Evans) watching Sam Wilson (Anthony Mackie) assuming the position to be the next Captain America in The Falcon and the Winter Soldier. After accomplishing his final mission in Avengers: Endgame, the original Star-Spangled Avenger chose his trusted ally and friend to be his successor as he fully retires from active duty. It took a while for Sam to embrace the privilege and responsibility of wielding the iconic shield given several interior and exterior factors — all of which were tackled properly in The Falcon and the Winter Soldier.
Steve didn't consider many of Falcon's personal issues — something that Bucky Barnes (Sebastian Stan) admitted to him in their Disney+ show. As revealed early on in The Falcon and the Winter Soldier, Sam was already dealing with personal financial issues. Learning about what happened to Isaiah Bradley (Carl Lumbly) - technically the first Black Captain America - also further discouraged him from becoming the next Captain America. During Sam's period of hesitancy, the U.S. government chose John Walker (Wyatt Russell) to be the next Star-Spangled Avenger. At first, he seemed like the perfect man for the job, but after it was clear that he doesn't deserve to continue the legacy of Steve, Steve's chosen heir ultimately accepted his inherited role.
The whereabouts of Steve during the events of The Falcon and the Winter Soldier is one of the biggest questions in the series. Marvel Studios seemed to purposefully leave this detail uncertain, motivating fans to come up with their own speculations to explain his absence. This inspired YouTube channel, DrMachakil who comes up with a fan-edit visualizing Steve watching Sam's journey to becoming the new Captain America on the sidelines. Watch the clip below:

As satisfying as Steve's ending was in Endgame, the specifics of his final mission remain a lingering mystery in the MCU. Even the filmmakers couldn't agree with what exactly went down during his last time-traveling adventure that led to him back in the late '40s where he reunited and lived a full life with Pegg Carter (Hayley Atwell). This could be an indication that even Marvel Studios is still unsure about a definitive explanation regarding the matter. In any case, they could simply leave this query unanswered since it doesn't really impact any of their upcoming narratives. That said, they should expect that fans would continue to discuss it until there's a canon clarification.
If Marvel Studios decides to finally address this particular loose plot point, they can use Smart Hulk (Mark Ruffalo) to explain what really happened. Aside from the fact that the Avenger understands the rules of time travel better than most other MCU characters, he was also there when Old Man Steve re-emerged at the end of Endgame. As seen in The Falcon and the Winter Soldier, Sam and Bucky may not be exactly keen on talking about it, but Smart Hulk might be more willing to discuss it.
